ARE YOU ELIGIBLE?
If you have a Psychosocial Disability as either a primary or secondary disability registered with the NDIS, or if you are already on the NDIS for another form of disability you can have a psychosocial disability added as a secondary disability.
REFERRAL
Contact Harmony Homecare about how to put in a referral. The Recovery Coach will work with you, your support coordinator and planner or local area coordinator to finalise the funding.
START WITH YOUR COACH
Sign service agreement, schedule of supports and start working with your coach to put together a recovery plan and start achieving your goals.
